Prior to your initial appointment, please fill out the pre-screen questionnaire, new client forms and consents online, 48 hours prior to your scheduled appointment.
Visits are billed automatically on the day prior to the appointment to the credit or debit card on file.
Medications will be refilled at each appointment. We do not offer emergency refill requests. Prescriptions will not be called in after operating business hours or during holidays.
If you miss your appointment, or if you do not cancel your appointment 24 hours prior to your scheduled appointment time ,you will be charged a $100 cancellation fee to the debit or credit card on file.
After you have completed your pre-screen questionnaire and offered an appointment, a link will be sent to your online secure client portal. At this time you are required to upload your ID, Insurance card if you are using insurance and a credit or debit card.
No controlled medications are prescribed at this time
